David Hallam: I wanted to share my experience I had with my wife’s grandma at Horizon Ridge. Our family didn’t know what to expect and when we got there we were so pleased. The facility was so new and clean the staff extremely friendly and our questions were answered concerning grandmas stay. The staff followed up and came through in a big way. I just wanted to thank everyone there for all the great work that they did.
lowell lowell: My mom 96 years old transfered to horizo. Ridge nursing and rehab center she was there for about a month I have to say this place was very nice the nurses and assistants were very nice to her and everyone visiting they showed alot of caring and compassion the job they do isn't easy and I couldn't do it for a dayThey are very special to do a tough job like they do every day they treated my mom sylvia very good and mom liked the food there it always seems fresh and good quality which was niceLovely and her other co-workers couldn't be better and even the rehab people were very nice just wanted everyone to know about horizon I'm sure most people only use reviews to complain but these caregivers are very good my mom and I both miss alot if the nurses and caregiversThankyou all again for treating my mom Sylvia so goodLowell katz
Summer Hernandez: The young girls and Evelyn with therapy were fantastic. Evelyn worked hand in ways that no therapist has before. When I came in my hand was closed shut after two days with Evelyn it started to loosen and I could open a little. Having good conversations with the ladies made my stay feel more natural. Being able to go outside for cigarettes was a big relief took a lot of stress off me.
Two Groovy: Me and my grandparents were practically part time residents because we have been here so much. My grandfather had been there for months and they treated us like FAMILY. One person in particular named Nate treated us so good he is now alert of this family. You can’t trust everyone with your loved ones but this is one place you can count on
Cortney Wade: I want to give my thanks and gratitude to Horizon Ridge and all the staff that work there. I read some of the reviews and wanted to write the review my grandmother experienced. Seeing that we were coming from the hospital, I was worried for her. But soon, my fears went away as the staff at Horizon Ridge quickly took charge and made the transition seamless even though the hospital didn't send everything they were supposed to. It was after visiting hours, but they allowed myself and my family to stay later since she just got there and made sure she was settled in. We had many visitors the next day from wound care to therapy, the doctor diatary nurses' activities, and the service department. She was given a care plan, and slowly, but surely, she got better. The staff was fantastic, the food was great, and soon enough, she was able to go home with therapy. The case team helped so much withthe discharge, and we couldn't be more grateful that she went there. Without this facility and the staff, our family knows she wouldn't be where she is today. She's home and back to normal daily living. Thanks so much to Horizon Ridge.

Sandy Finney: I love this place. I was there for 2 months. Recovering from a fractured ankle. I had to learn to walk on one leg. PT And OT. Help me with tools that I can use even at home. They also had my back when it came to not being discharged before the right time. I Love how they interact with each other. Always a good time.Now for the incredible nurses. Always have a smile on their faces. Regardless of how their day may have been. I never had to wait a long period for my meds. I also want to say Thank you to the laundry person and the maintenance people. They were always making sure my clothes were cleaned and back to me on time. Maintenance made sure my room was clean. Patrica, you always have a smile on your face. And engaging with the patients. If I miss anyone I'm sorry. I miss you all. I will come and visit soon.❤️🌹🌷I have pictures but I can not upload them.😩P.s. I purposely did not mention the food. It’s horrible. That needs to be fixed. No favors!! Needs to be fresh.
Alma Manzo: Coming to this facility was one of the best decisions we made as a family. We had many options of places to go and Horizon Ridge was our choice. The staff was very friendly and one step ahead of my grandfathers care. I want to thank everyone involved for their help and I highly recommend Horizon Ridge as THE place to take there loved ones after surgery.

This was our second time having to go to a rehabilitation center and the first one I won’t even name.Horizon staff from therapy to housekeeping activities and the CNAs were so amazing and they made this difficult process bearable. Thank you to everyone at this facility forgetting our grandma to come home strong and safer than ever.
Starla V: I feel so lucky & blessed to receive rehab therapy at Horizon Ridge Nursing facility. I was bedridden and unable to walk after abdominal surgery. I was I'm bed for a total of 4 months in 4 different facilities, but this one made the difference.The staff here is phenominal! All the physical therapist make you want to get better. I wasn't sure I was going to walk again. But sure enough. Thanks to the marvelous staff of doctors, nurses , and therapists. I am discharged and walking out! Even the food was good; no kidding! Normally hospital food is barely edible! Lol 🤣Seriously, I cannot say enough about Horizon Ridge. They literally helped me get my life back. My husband and I are in our 70's and if we have any more surgeries which need rehab there is no question where we're going.

All the way from the top employee we felt empathy compassion and we knew our grandfather was at the right place. Judy Logan: All the staff at Peak Dr, Northwest Clinic, are warm, caring, professional people beginning with the women at the front desk, who are always accommodating and smiling, to the myriad clinicians and ancillary helpers in the therapy rooms evaluating and- assisting patients.The facility is small, but the excellent therapists, et al, manage my treatment activities successfully, even as other patients cycle through In successive rounds.There are no traffic jams, road blocks, or road rage incidents, only a peaceful flow of people and caring attendants moving through a prescribed regimen for each patientConstant oxygen and blood pressure monitoring, as well as observation by the staff, also ensure patient well-being during the treatments and afterward.I have “malignant blood pressure”, which means my blood pressure can and does soar quickly and inexplicably, making me very dizzy, nauseous and in danger of a stroke or heart attackOnce, when this erratic behavior started, my primary therapist, Jason, took the right steps to stop the treatment activity, make me rest and drink plenty of fluids, and monitor my blood pressure several times until it came down to an acceptable level.He then suggested that I go home, rest, and followup with my Primary care provider.He did a terrific job addressing a potentially dangerous situation without prescribing, or practicing medicine - only good Respiratory Therapist skills.i

Nicole Trincilla: , the majority of people that work there are very caring wonderful people.I wanna start with the nurses like Roger and Deb and Donna and great caring physical therapists like Roy and Val and wound care Lorma and her sidekick really working hard in trying to get my dad's wounds cleared up. These actually almost killed my dad, it was almost down to the bone and these people were really tried so hard and did a great job getting him in his way I really want to thank everybody including Victoria and Deedee and Donna and even the sweet girl Ashley in front who helps whenever and wherever she can and sweet Roberto and Miss Lakeisha Gray who is an ANGEL on Earth. It is not easy running a rehab facility and I know that this place is not perfect by no means but I would have 0 problem having my dad back in this place again. There are very very caring good people here and that is a good place to start.
